Assisted Living Services/Supportive Housing

 Our Assisted Living/Supportive Housing program is a free, publicly funded, community support service that provides eligible high-risk seniors with personal support services on a scheduled and unscheduled basis.  With funding from Ontario Health, Legion Village operates as a Health Service Provider to deliver Supportive Housing under the Assisted Living Services Policy, 2023. This entails providing a combination of housing assistance and services to enable people to live as independently in their homes for as long as possible. To qualify, individuals must be able to transfer independently or with minimal support.  To determine eligibility, complete the application form and submit it either in person or online.   Please note that space is limited and eligible residents will be placed on a waiting list.

 The Assisted Living/Supportive Housing program is not a medical program and does not replace regular primary health care or routine nursing monitoring.  We provide a specific level of care tailored to seniors who require some assistance with daily living but do not need full-time medical care.  We are not equipped to provide the intensive medical/nursing services available in long-term care homes, retirement homes, or hospitals.

Resident Bill of Rights

All Residents of the Legion Village Supportive Housing program have the right to:

  1. Service provided in a respectful manner and to be free from physical, sexual, mental, emotional, verbal and financial abuse by the provider.
  2. Service provided in a manner that respects your dignity and privacy and that promotes the resident’s autonomy and participation in decision-making.
  3. Service provided in a manner that recognizes your individuality and that is sensitive to and responds to the resident’s needs and preferences, including preferences based on ethnic, spiritual, linguistic, familial and cultural factors.
  4. Receive services free from discrimination on any ground of discrimination prohibited by the Human Rights Code or the Canadian Charter of Rights and Freedoms.
  5. A resident who is First Nations, Metis or Inuk has the right to receive home and community care services in a culturally safe manner.
  6. Receive clear and accessible information about their home and community care services.
  7. Participate in the assessment of your needs, development of their care plan, reassessment of their needs and revision of their care plan.
  8. Designate a person to be present with you during assessments and to participate in the development, evaluation and revision of their care plan.
  9. Receive assistance in co-ordinating your services from the health service provider or Ontario Health Team.
  10. Give or refuse consent to the provision of any home and community care service.
  11. Raise concerns or recommend changes in connection with the home and community care service provided to you and in connection with policies and decisions that affect your interests, to the provider of the home and community care service, the Agency and its employees, government officials or any other person, without fear of interference, coercion, discrimination or reprisal.
  12. Be informed of the laws, rules and policies affecting the operation of the provider of the home and community care service, including this Bill of Rights, and to be informed, in writing, of the procedures for initiating complaints about the provider.
Resident Responsibilities

Legion Village has adopted the Ontario Health atHome Patient Responsibilities set out below¹.

All Residents of the Legion Village Supportive Housing program have the right to:

  1. Treat all service providers including Legion Village staff with courtesy and respect; free from discrimination, harassment and acts of violence (e.g. yelling, name calling, threats). Ontario Health atHome and Legion Village will not accept discrimination that violates a staff member’s rights and safety;
  2. Provide a safe working environment for all service providers by:
    1. Providing a smoke and vape-free environment;
    2. Securing pets during visits when requested;
    3. Ensuring clear hallways free of clutter and fall hazards; and
    4. Securing all illegal and potentially harmful materials.
  3. Participate in developing your care plan;
  4. Keep your care coordinator and/or Legion Village staff informed about any changes to your health status and/or support system;
  5. Be available and prepared to receive service;
  6. Inform your care coordinator and/or Legion Village staff in advance if you are not available to receive service;
  7. Inform us of any changes to you or your emergency contact’s contact information;
  8. Work together with us to identify and resolve any concerns related to your care.

Both the resident, and Legion Village staff share a joint responsibility to ensure your rights are met. However, under certain conditions, the Legion Village staff have the right to leave at any time should they feel unsafe.
